I had the unique opportunity this past weekend to visit Dr. Charles Runels in beautiful Fairhope, Alabama. Dr. Runels is the creator of the famous “Vampire Facelift (TM)”. The day with Dr. Runels and his assistant Laura left me with a great impression. Many of my colleagues perceive Dr. Runels from his videos and website as being a “fringe” physician. Nothing could be further from the truth. Although his personal style (which is seen on his YouTube videos) and passionate desire to protect the quality of his creation “The Vampire Facelift (TM) can be interpreted as “strange”, my feeling is that he is an exceptionally creative and caring physician whose mission is help people live life to their fullest potential.

Plastic Fantastic: Platelet Rich Plasma fillers

Between an eye-lift and a hair transplant, Dr Prasad took some time out of his busy schedule to talk to Swide about Platelet Rich Plasma (PRP) fillers, the latest must have age defying practice.

But first here’s a brief biology lesson to get to grips with the terminology. Plasma is the yellow liquid which makes up over 50% of the blood. The actual blood cells are suspended in the plasma, making it the petrol of our organism. Platelets are those tiny cells that serve to create blood clots and therefore start the healing process of the skin.

PRP has been used for a while in other medical specialities, such as in orthopaedics for the cure of joints and chronic injuries to help stimulate the healing process. While three years ago, medical professionals from the cosmetic sphere started to think about the uses of PRP in their field. Dr Prasad, a facial rejuvination specialist at the Prasad Cosmetic Surgery Clinic in New York City, has been using this substance as a filler for two years now. “By placing it [PRP] in the skin” Dr Prasad explains, “you are injuring the skin in a controlled way and you are essentially healing a wrinkle by filling it with a material that will recruit the body’s collagen and start the healing process.” In layman’s terms, unlike Botox, PRP is used to restore volume under the skin.

PRP fillers are particularly effective on fine lines around the mouth, eyes, even bags under the eyes and also to reduce the depth of acne scars. The optimum age to undergo a treatment is when you are over forty, unless you want to treat acne scars, then the median age of the patient is halved.

And how about those stubborn expression lines? Well PRP fillers can also work on those. Dr Prasad says that this treatment can be used“ anywhere where the lines are relatively small, forehead lines are shallow, Botox can relax them but there is no good filler to plump them out and we’ve been using PRP.”

How do PRP fillers compare to more traditional chemical fillers like Restylane? Dr Prasad explains that it’s a matter of patience, the two provide more or less, equivalent results, its just a question of time. “With Restylane you get immediate results, because 1cc of liquid equals 1cc of correction. With PRP you may get immediate appearance of improvement but you have to wait to see the result because body has to respond by generating collagen. It’s a healing process.” Even though the results may come more slowly, the effects of PRPs however, can last a year or more, while other chemical fillers start fading after about six months. A PRP session, which can consist of 3 or 4 injections, is also better value for money, as typically it costs $1.500, while for one syringe of Restylane the retail price is $800.

PRP fillers are attracting an increasing amount of would be clients. “People do a lot of research, they learn a lot online, and they come and ask about it.” Dr Prasad has found that his role as surgeon has developed into teacher as well, “ often I have to educate them on what [fillers] can and can’t do. It takes the judgement of a professional to understand what they can do and how to apply them so you get the maximum benefit.”

In recent years, cinema, TV and literature have resurfaced the public’s fascination and interest in Vampires- today, in cosmetic surgery, the benefits of blood go further than fantasy and are becoming reality.

Pelleve Skin Tightening: A new procedure, with LI roots, in the war on wrinkles (Newsday)

There’s yet another weapon in the war on wrinkles.

Called Pelleve, the process uses radio-wave technology to induce heat into the skin, stimulating collagen growth to tighten skin and reduce the appearance of wrinkles.

The technology was developed by Ellman International, a medical company in Oceanside, N.Y., that got its start making equipment for dental surgery.

Pelleve was granted FDA approval as a facial wrinkle reducer in May 2009, said Tom Harper, vice president of global marketing at Ellman. The equipment was initially used to stop bleeding during surgery, said Harper, adding that the treatment is now being used in well over 200 facilities in the country. “Patients seem to feel there’s a lot of value in the product, especially around the eyes and jawline,” he said.

WHAT TO EXPECT The doctor or technician will apply a protective gel, then run the handpiece over the area being treated. Patients generally report feeling a sensation of warmth. “It goes to a point where it feels very hot,” said cosmetic surgeon Amiya Prasad, one of several Long Island doctors using the treatment. “Most people tolerate it very well,” he added. No numbing or anesthesia is required (or desired since they want you to speak up if it gets too hot).

DOES IT WORK? “Pelleve has worked out to be a good service for our patients, for the person who is not quite ready for a face-lift,” Prasad said. Likewise, Greenvale dermatologist Deborah S. Sarnoff says patients have been happy with the procedure, which she often combines with Botox and fillers. “A Pelleve treatment is administered to improve overall skin laxity,” she said, “then I inject filler to areas that may need more volume and Botox to relax the dynamic wrinkles.”

Sarnoff says that, unlike other treatments, “patients see an immediate improvement,” though many doctors suggest two or three sessions for the best results.

HOW’S IT DIFFERENT? Pelleve differs from Thermage, another procedure that uses radio frequency, in that the handpiece can be moved back and forth over the skin, which doctors say allows them to more easily target problem areas.

COST Harper says costs vary widely. In this area, treating the entire face runs $1,500 to $2,000 a session; treating a single area (like the eyes) can run $500 to $1,000.

WHO SHOULD HAVE IT? Both Prasad and Sarnoff say the ideal Pelleve patient is someone with mild to moderate wrinkling or skin laxity, or someone who had a face-lift and wants to maintain the results.

Plan Now to “Face” the Holidays … Here’s my Seasonal Timetable for Personal Facial Rejuvenation.  With the holiday season fast approaching, I’d like to remind you that October through mid-November as the ideal time to rejuvenate your face for the holidays – not just to look better, but because with all the emotional stress the holidays can bring, looking your best can be an incredible confidence boost.

For many people, what should be a joyous season of fun, family and fellowship becomes a season of foreboding instead.  School plays, office parties, winter weddings, family portraits, social activities, holiday dinners, shopping, and religious events all put demands on individuals – including demands to look their best.  From mid-November through New Year’s Day, the stress-filled list of “must-attend” soirees and celebrations can feel endless.

I see it every year.  When you become overwhelmed by these social obligations, the holiday blues can start to set in.  The result?  You start to feel tired and stressed, your confidence can slip and self-doubts can surface. Gazing in the mirror and noticing a few extra laugh lines, frown lines, puffiness in the eyes – or even slight sagging at the jaw line or neck – all of these only make the thought of socializing during the holiday season even more agonizing.

Every year, I’ve witnessed an increase in surgical and non-surgical treatments during this time of year – and, for a lot of reasons, this is a good thing.  Men and women want to present their ‘best’ at this time of year, and each year thousands of them realize that – when facing activities they are obligated to attend, they want the added confidence that a facial rejuvenation procedure delivers.

However, if you are considering a facial rejuvenation, scheduling the procedure well in advance of the holiday is important, both to allow full healing and to let the added self-confidence that naturally comes with a facial rejuvenation procedure to “sink in.”

Facelifts – Forget the Six-week Healing Process

The healing process for a traditional facelift takes up to 6 weeks – sometimes longer. However, with the minimally-invasive surgical techniques I have pioneered, the recuperation time for my natural-appearance facelift is diminished considerably – generally to 2-to-3 weeks.

Four Weeks Before The Event

If you are looking for a Sculptra session to fill in the minor fatty tissues that, when lost result in wrinkles, or if you want to minimize wrinkles with the Pelleve process, book these treatments at least four weeks before your first event.

Two Weeks Out

Even just a few weeks before the festivities begin, eyelid rejuvenation, cheek implants, brow lifts, and fractional CO2 laser treatment– which lessens the visibility of wrinkles, scars and discoloration – can be performed, and recovery completed.  Book these no later than two weeks before your first event.

One Week and Counting!

With just a week to go, either a light chemical peel – or the use of injectibles – is the best way to combat those laugh and frown lines. There are many types of injectibles, each targeting a different part of the face. Botox focuses on the brow bone area and around the eye, fighting frown lines found between the eyebrows. Restylane is used for diminishing wrinkles and replenishing areas lacking firmness.  Juvederm is used for reducing wrinkles and used as a filler for facial areas and lips.  Finally, Radiesse is also used for lessening wrinkles. Injectibles should be given a week before you expect to see effective results.
